Summary:
The Nurse Practitioner is a healthcare provider who, working collaboratively within a multi-disciplinary health team, is responsible for providing comprehensive care to acutely ill patients in the hospital setting under the direction of a supervising physician. Provides care to patients on the North Oaks Hospital Medicine service as assigned, and under the direction of the Medical Director and other collaborative agreement physicians. Provides care according to established policies, protocols, bylaws, and rules and regulations of the medical center and medical staff. Seeks opportunities for professional growth through participation in continuing education, staff development and hospital committees.

1. Must be credentialed and approved for privileges through the established Medical Staff Committees initially and on an ongoing basis.2. Specialized or Technical Education Required:Must hold current Nurse Practitioner licensure by the Louisiana State Board of Nursing.Graduate of an accredited school of nursing.Meets for allied health professionals as established under medical staff bylaws.3. Responsibilities:
1. Conducts a comprehensive physical health assessment of patients with acute health problems which may include the psychosocial, cognitive and personal assets and limitations of the patient/family.2. Establishes medical diagnoses; orders and evaluates diagnostic studies; performs and monitors therapeutic procedures; assesses and/or recommends follow-up plans under the direction of a supervising physician.3. Records and documents health appraisal data necessary to maintain, coordinate and/or expedite required services.4. Assumes responsibility and accountability for time management, professional relationships and interpersonal interactions.5. Acts as a resource to healthcare staff and supports educational programs to improve management of patients/families.6. Ensures a high level of quality care and services for patients by planning, implementing and communicating treatment plans in collaboration with medical staff and other members of the interdisciplinary healthcare team.7. Assist the medical staff with admissions, discharges, and triage of patients as needed, and as defined in the allied health standards of practice.8. Interfaces with family and interdisciplinary team members as needed. Educates patients and/or their families in order to promote wellness, prevent health problems, and intervene in acute illness.9. Promotes superior customer service by serving as a role model; maintaining good working relationships with patients, visitors, physicians, and employees; promoting a sense of teamwork within the program; and timely follow-up with patients, family members, physicians and staff.10. Works with program assistant and medical director to ensure accurate and efficient coding for services provided to assigned patients.11. Maintains and enhances a current level of knowledge relative to advanced nursing professional practice by maintaining professional affiliations and keeping abreast of the latest practice trends, as well as continuing education requirements necessary for licensure.12. Is able to demonstrate competency in age specific criteria as applicable to job scope.13. Performs approved procedures competently, per credentialing approval.14. Documents in the medical record according to policy and medical staff rules and regulations, with regards to content, notes, orders, security and confidentiality.15. Adheres to medical staff bylaws as applicable to advanced practice nursing under allied health policies.16. Follows North Oaks Health System’s Compliance Program and Federal and State Regulatory guidelines.
